Pet Boarding Policy Form
Thank you for choosing Toby’s Grooming Place for your pet’s boarding needs! Please review the following policies carefully to ensure a smooth experience for you and your pet. After scroll down to sign the form. Thank you for your understanding and cooperation!
Drop-Off and Pick-Up Policy
We are flexible when it comes to drop off and pickup times. We can intake your pup as early as 7 am, and pickup at 9 pm. We do kindly ask that you let us know of your intended drop off and drop off times.
Deposit and Cancellation
For all pet boarding stays, we require a 25% non-refundable, non-transferable deposit to lock in your fur babies stay. This amount is of course discounted from your total amount due on the day of your pup's pickup.
If you need to cancel, the deposit can not be refunded or transferred to another pet boarding stay.
Pricing
Day Rate:
$35/ Day per Dog for current grooming clients
$50/ Day per Dog for non-grooming clients**
**Non-grooming clients are clients who have not had their pup's groomed with us at all, or have not groomed their pup's with us in 6+ months
Payment Options
We currently accept Venmo, Zelle, CashApp, and AppleCash for your deposit.
And cash, Venmo, Zelle, CashApp, and AppleCash for your final payment on the day of pickup. Payments can be sent via the phone number registered to Nazaret Uribe (863-617-4617). We also have custom links/QR codes for all methods. Just ask, and we can provide them to you.
Food and Doggie Bed
Please provide detailed feeding instructions, including portion sizes, and feeding times. Food should be provided in labeled, resealable containers.
No doggie bed or crate is required. Toby's Grooming Place can provide these. However, if yo would like to provide a blanket, toys and anything else that will make the pup feel more at home, it is welcomed.
Health and Emergency Policy
We will not accept pets who are sick or recovering from surgery without prior notice and approval. If your pet becomes ill during their stay, we will contact you immediately. In the event of an emergency, we will contact your veterinarian and/or seek emergency veterinary care. All associated costs will be the responsibility of the owner.
By signing below, you agree to release Toby’s Grooming Place LLC, its employees, and caretakers from any liability related to unforeseen incidents or emergencies during your pet’s stay.
Vaccination Record
All pets must have their current core vaccinations before their pet boarding stay, including the Rabies shot.
Clients must provide proof of the vaccination record, which can be attached to this form.
